Go back and recheck timing at TDC physically and check rotor position. If you have points regap them and recheck all the spark plugs to make sure they are gaped correct. Dont use Botch or any German type platinum plugs. They dont work well in Hemi's TRUST ME!!! Use AGR-42 Spark plugs. If all of the above is good to go. Check your Compression on each Cyl. The rings may NOT been offset on the piston when it was rebuilt or the head torque needs to be checked for blow-by. You could have a simple intake vacum leak or crack on the intake. Good Luck! I Had the same problem once and ended up changing the Intake and Carb.
